Referral Process

Veterans and their families will choose their own therapist from a directory of therapists on the website. If a therapist is not taking new clients, the veteran and family are referred back to the website.

How to Become a Member of the Project

To begin the application process, please review, print and sign the Guidelines and then complete the Online Form. If you have any questions, please contact us by e-mail or phone.

  1. Along with your completed application, or separately if applying online, please send a copy of your current malpractice insurance, license, and a signed copy of the Guidelines to:

  2. If you would like your photo on your listing, please also include on a disk, or email a .jpg or other standard image file to info@returningveterans.com.

Note: The information you put on the application form will be posted on the website. Should you decide at any time that you would not like to participate, please notify RVRPNW by calling (503) 402-1717 or email to info@returningveterans.com. Your name will then be removed from the website.
